The mission of the Caregivers Conference is to provide a Jewish perspective to caregivers, family members, lay leaders, clergy and professionals by sharing practical approaches for caregiving, tactics to “take care of” the caregiver, plus educational and research updates about dementia. The conference is designed to support caregivers and let them know that they are not alone on their journey. The conference is presented in collaboration with Jewish Family Service of St. Paul.

The professional staff works closely with adults age 60 and older and their families to ensure safe, supported and independent living at home. Participants can receive care coordination, caregiver coaching or care plan consultations. In addition, clients have access to other services such as transportation, accompanied transportation, shopping assistance, senior companions, Survivor Support, Kosher Meals on Wheels, Memory Café and our Powerful Tools for Caregivers class.

We offer several programs and services for older adults to help them remain in their homes and to live as independently as possible. Family consultations with a nurse and/or social worker are available with access to JFS mental health professionals as needed. Concerns that can be addressed include guidance with making long-term care decisions and plan; finding resources and support; deciding if it’s time to make a move to a more supported setting; and addressing disagreements in the family regarding roles and responsibilities for care. Staff includes bilingual Russian-English speaking case managers.

Advance care planning helps people make decisions about the health care they would want to receive if they become unable to speak for themselves and includes sharing your personal values with your loved ones; getting information on the types of life-sustaining treatments that are available; deciding what type of treatment you would want should you be unable to speak for yourself; completing advance care directives to put into writing what types of treatment you would or would not want, often referred to as a living will, and who you choose to speak for you; and designating a health care power of attorney.
Advance care planning can help alleviate unnecessary suffering, improve quality of life and provide better understanding of the decision-making challenges facing the individual and their caregivers. An advance care plan can be used at any stage of life. Work with a professional who has expertise and extensive training. They have provided countless hours of compassionate care to many people at the end of their life. They will listen to you. They will show respect and dignity to your culture and its practices. They will guide you through the completion of all necessary documents to ensure your wishes are respected and followed. They will work with and counsel your family to the degree you choose.

JFS staff provides education and training to groups surrounding the issue of dementia. The program raises awareness in the general community about the disease and provides tools for positive interactions between members of the community and people with dementia.

High-quality, personalized assistance to you or a loved one. Professionals with extensive experience in aging issues (nurses, social workers, and/or mental health practitioners) work with you and your loved one to develop individualized care plans based on in-home, comprehensive assessments. Staff provides ongoing monitoring of the plan with phone calls and visits to ensure that the needs of your loved one and family members are met. This includes providing communication with out-of-town care partners and family members to keep everyone involved.

The Capp Center St. Paul supportive services improve the quality of life of individuals and help them remain safely in their homes and community for as long as possible. The JCC offers transportation services to individuals with limited access to transportation. We provide services M-F for medical appointments, errands and transportation to the JCC. Rides are provided based on a sliding fee scale and availability. The JCC is proud to provide the only kosher Meals on Wheels service in St. Paul. Kosher Meals on Wheels is a partnership with Jewish Family Service of St. Paul (JFS) and Sholom Alliance. Plus, the JCC employs eight Russian-speaking medical interpreters that provide both transportation and interpreting for medical visits. Interpreting is provided.

Sholom’s roots began in 1908 when the first Jewish Home for the Aged opened in St. Paul.
